Our Story: Both my co-founder and I have had personal experiences with family members and friends facing serious illnesses. Through these challenges, we noticed a recurring issue - patients often felt lost and disconnected from the necessary support after leaving the hospital. This disconnect not only worsened their conditions but also created inconvenience at best. Determined to make a positive impact, we embarked on a mission to improve the status quo and enhance post-acute care. Who We Are: We are a dynamic and innovative team, fueled by a shared vision to bridge the gap between acute care and community support. We firmly believe that a patient's journey to recovery should be seamless, compassionate, and supported at every step.
